These drugs are not capable of discriminating between cancer and other healthy cells and so the drugs which are cytotoxic destroy both types of cells resulting in very dangerous side effects which may be worse than the cancer itself. The principles of cyclo-oxygenase and cox selectivity comes to mind. Scientists have not yet discovered a conventional drug that is cox selective. Once this is achieved, most health problems would be solved.<\/p>\n<p>The point I am trying to present is better illustrated by reference to the abstract from a book written by a renowned scientist. The title of the book is CONCEPTS AND MISCONCEPTIONS OF DRUGS DISCOVERY. The abstract is reproduced below:<\/p>\n<p>Many new technologies start as science fiction ideas. The \u201cideal drug\u201d that Paul Ehrlich proposed in 1906 was based on a clear definition of its purpose\u2014to be 100% effective all the time and have no undesirable side effects. Much effort has been expended in this direction, especially in the last 50 years or so, but this concept of a \u201cmagic bullet\u201d remains only science fiction.<\/p>\n<p>A fundamental law of physics states that \u201c\u2026only light of energy that can cause transitions from one existing energy level to another will be absorbed\u201d. A similar rule applies to the action of drugs\u2014for a drug to act on a given disease, it needs to interact or bind with the molecular structures associated with the disease, and elicit a therapeutic effect. Unfortunately, in most cases, the same structures to which the drug can bind are present not only in the disease, but also in normal cells; this makes it obvious why delivering drugs solely to the sites of a disease is important.<\/p>\n<p>We expect all drugs to be beneficial and not harmful. Unfortunately, this is not very often the case. Here is why.<\/p>\n<p>The ultimate task of drug delivery is to induce drug action only on diseased cells (for example, cancer cells). Such a system must avoid the body\u2019s natural defense mechanisms and deposit the drug at its intended site of action. From that point on, the fate of the drug is determined by its own physicochemical and biological properties. Only drugs with the appropriate pharmacokinetic properties will benefit from the application of an appropriate delivery system.<\/p>\n<p>Why is targeting the disease important? Let us take cancer as an example.<\/p>\n<p>Cancer is the second most lethal disease in the US: some 1.7 million Americans were diagnosed with cancer in 2018, of whom around 600,000 died. Over 15 million Americans cancer survivors are alive today.<\/p>\n<ul>\n<li>Cancer medicine generates enormous revenues but marginal benefits for patients;<\/li>\n<li>Spending on cancer care increased from $125 billion in 2010 to\u00a0<a href=\"https:\/\/www.valueinhealthjournal.com\/article\/S1098-3015(16)30444-2\/fulltext\">$175 billion in 2020<\/a>;<\/li>\n<li>Progress in treating cancer is frequently described in terms that are not realistic, such as \u201cmaking cancer history\u201d;<\/li>\n<li>No one is winning the war on cancer. Some even argue that advances are \u201cmostly hype, the same rhetoric from the same self-important voices for the past half-century\u201d;<\/li>\n<li>Successful therapies have indeed been developed for specific blood, bone marrow, and lymph cancers. Treating solid tumors remains largely unsuccessful, however;<\/li>\n<li>The age-adjusted mortality rate for all cancers in the US in 2020 was about the same as it was in 1930;<\/li>\n<li>Statistics show that, without reductions in smoking, there would have been virtually no reduction in overall cancer mortality in either men or women since the early 1990s;<\/li>\n<\/ul>\n<p>72 new anticancer drugs approved by the FDA between 2004 and 2014 prolonged survival for an average of 2.1 months. Published literature suggests that \u201c<a href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/2463590#:~:text=Our%20results%20show%20that%20most,poor%20correlations%20with%20overall%20survival.\">most cancer drug approvals have not been shown to, or do not, improve clinically relevant endpoints<\/a>\u201d, including survival and quality of life. As such, there is a concern that that \u201c<a href=\"https:\/\/www.cambridgescholars.com\/news\/item\/the%20FDA%20may%20be%20approving%20many%20costly,%20toxic%20drugs%20that%20do%20not%20improve%20overall%20survival\">the FDA may be approving many costly, toxic drugs that do not improve overall survival.<\/a>\u201d<\/p>\n<p>All of the above issues should be considered when attempts are undertaken to develop new, disease-targeted drugs. In addition, the limits of medical arts and the Hippocratic Oath, \u201cFirst, do no harm\u201d, need to be recognized.<\/p>\n<p>Nearly all administered drugs distribute generally throughout the body and bind to their target of action in any location they reach, regardless of whether the target is associated with a disease or with healthy cells. The need for controlling the distribution of drugs in the body and its pharmacokinetics gave rise to the field of drug delivery, which explores approaches that control the manner in which drugs enter the body (transdermal, oral, buccal, etc.), distribute within the body (such as by being attached to an inert macromolecule or a particle, etc.), and are released (for example, by enzymatic cleavage of a covalent bond, change of pH, etc.). So-called controlled drug-delivery systems started to be developed in the 1950s and have been relatively useful and successful.<\/p>\n<p>The concept of \u201cdrug targeting\u201d followed later, attempting to put into practice\u00a0<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2790789\/\">Ehrlich\u2019s idea that a drug molecule should accumulate in the target organ\u00a0<\/a>or tissue selectively to reach the pharmacodynamic concentration of the drug at the disease site while its concentration in nontarget organs and tissues remains low, preferably below the toxicity level. However, effective disease-site-specific delivery systems have not yet been developed, not even when antibodies were used as drug carriers. The term \u201ctargeted\u201d is often used to describe various new drugs and therapies with the intention to suggest that they exhibit higher specificity in treating the disease. The reality, however, is that the use of all such recently denoted drugs is associated with a large number of often very serious and undesirable side effects. Using the term \u201ctargeted\u201d when it relates to the intent of what the drug is to do, and ignoring the fact that its action is generally distributed throughout the body rather than focused on the locus of the disease, is misleading.<\/p>\n<p>After more than 100 years, much research and many claims for promising new targeted drug-delivery platforms under development, specific disease-targeting systems have not yet reached effective clinical application.<\/p>\n<p>The key to developing effective targeted drugs is to understand the mechanisms of the disease that is to be treated. In particular, defects in molecular and cellular processes that become the starting points of specific pathologies need to be known. The mechanisms underlying human diseases most often relate to biological processes that are strongly conserved through evolution, such as cell communication, signal transduction, metabolism, inflammation, and immunity. These processes rely on interactions between multiple cell types, tissues, and organs of the whole organism. Animal models of disease are invaluable tools to study the complexity of disease phenotypes, but it must be kept in mind that none of the available models represents human diseases exactly.<\/p>\n<p>The naivet\u00e9 of paradigms proposed for site-specific targeting of drugs stands in great contrast to the sophistication and complexity of the life-supporting biological system.\u00a0<a href=\"https:\/\/www.cambridgescholars.com\/product\/978-1-5275-5584-6\">This book<\/a>\u00a0presents a paradigm that is based on a comprehensive understanding of human disease at the molecular levels, and uses this knowledge to identify and target new drugs to unique molecular structures associated with diseases. In addition, it illustrates the complexity of determining molecular targets through the use of various examples.<\/p>\n<p>Since the mechanisms of diseases are often not fully understood, it becomes very difficult to determine exactly which molecules are involved and can be used as targets for drug action. Drugs acting on the target in a disease-modifying manner would change the molecular and subsequently the physiological processes associated with the disease. The vast majority of current drugs act on targets associated with the disease as well as normal cells, a situation that generates extensive and often serious side effects.<\/p>\n<p>New information on diseases and drug delivery appears almost daily, and is very often described as \u201cpromising\u201d. Here is a particularly recent example:\u00a0<a href=\"https:\/\/www.cardiff.ac.uk\/news\/view\/1749599-discovery-of-new-t-cell-raises-prospect-of-universal-cancer-therapy\">a new part of our immune system was recently discovered at the University of Cardiff, UK<\/a>, and it was claimed that it could be harnessed to treat all cancers. The discovery, a T-cell that in a lab setting killed a range of cancer cells, (lung, skin, blood, colon, breast, bone, prostate, ovarian, kidneys, and cervical) has not been tested in vivo. However, a particular T-cell receptor interacts with the MR1 structure that is present on the surface of every cell in the human body. Developing disease-targeting based on a target present in all cells may be problematic. There are many ways in which cells can be killed in a test tube, most of which do not transfer fully across to the treatment of humans.<\/p>\n<p>The prevailing paradigm in drug discovery is to design ligands with maximum selectivity to act on individual drug targets. The major flaw of this paradigm is the fact that the targets selected are present not only in disease cells, but also in healthy cells. As such, a new paradigm needs to be adopted. It should:<\/p>\n<ul>\n<li>identify unique molecular structures present on a narrowly defined population of cells to evaluate whether finding such features might be at all realistic;<\/li>\n<li>use Artificial Intelligence matching human \u201cbrainpower\u201d to set up an effective algorithm for identifying such unique structures relevant to diseases.<\/li>\n<\/ul>\n<p>To this end, AI will need to be able to generate new knowledge and know-how, speculate, imagine, reason, be logical where humans often cannot be, handle the \u201cwhat if\u201d questions, predict, and make decisions.<\/p>\n<p><a href=\"https:\/\/www.cambridgescholars.com\/resources\/pdfs\/978-1-5275-5584-6-sample.pdf\">https:\/\/www.cambridgescholars.com\/resources\/pdfs\/978-1-5275-5584-6-sample.pdf<\/a><\/p>\n<p><b>Karel Petrak<\/b>, DPhil, MSc, has spent some 40 years in the pharmaceutical industry, achieving success in pharmaceutical and biotechnology drug development, drug discovery and delivery of conventional drugs, proteins, peptides, and genes, and in translational research. A recent journal publication revealed that polyphenols from certain accessions of Sorghum Grain had cytostatic and not cytotoxic effects on some Cancer cell lines and it was suggested that consumption of some types of Sorghum bicolor grains would have chemopreventive activities. I will reproduce the abstract of the article below:<\/p>\n<p><b>Sorghum Phenolic Compounds Are Associated with Cell Growth Inhibition through Cell Cycle Arrest and Apoptosis in Human Hepatocarcinoma and Colorectal Adenocarcinoma Cells<\/b><\/p>\n<p>&nbsp;<\/p>\n<p><b>Abstract<\/b><\/p>\n<p>Phenolic compounds in some specialty sorghums have been associated with cancer prevention. However, direct evidence and the underlying mechanisms for this are mostly unknown. In this study, phenolics were extracted from 13 selected sorghum accessions with black pericarp while F10000 hybrid with white pericarp was used as a control, and cell growth inhibition was studied in hepatocarcinoma HepG2 and colorectal adenocarcinoma Caco-2 cells. Total phenolic contents of the 13 high phenolic grains, as determined by Folin\u2013Ciocalteu, were 30\u201364 mg GAE\/g DW in the phenolic extracts of various accessions compared with the control F10000 at 2 mg GAE\/g DW. Treatment of HepG2 with the extracted phenolics at 0\u2013200 \u03bcM GAE up to 72 h resulted in a dose- and time-dependent reduction in cell numbers. The values of IC50varied from 85 to 221 mg DW\/mL while the control of F10000 was 1275 mg DW\/mL. The underlying mechanisms were further examined using the highest phenolic content of PI329694 and the lowest IC50 of PI570481, resulting in a non-cytotoxic decrease in cell number that was significantly correlated with increased cell cycle arrest at G2\/M and apoptotic cells in both HepG2 and Caco-2 cells. Taken together, these results indicated, for the first time, that inhibition of either HepG2 or Caco-2 cell growth by phenolic extracts from 13 selected sorghum accessions was due to cytostatic and apoptotic but not cytotoxic mechanisms, suggesting some specialty sorghums are a valuable, functional food, providing sustainable phenolics for potential cancer prevention.\u00a0<a href=\"https:\/\/www.mdpi.com\/2304-8158\/10\/5\/993\/htm\">View Full-Text<\/a>.<\/p>\n<p>To confirm that Sorghum bicolor may be relevant for finding solution to the intractable problems of cancer, the NATIONAL CANCER INSTITUTE recently announced that Sorghum bicolor supplement has been included in their Dictionary of drugs and described it as being rich in Polyphenols and phenolic acids and has the potential for antioxidant, anti-inflammatory, immunomodulating and chemopreventive capabilities.
